SERVICE AGREEMENT First Delivery: I understand that there will be a $15.00 set-up fee payable on my first delivery. Cancellation/Change/Hold Scheduled Delivery: I understand that any and all changes or holds must be made ONE WEEK prior and cancellations must be made TWO WEEKS prior to my scheduled delivery or I will be charged for the delivery. Damages: I understand that there will be a $10.00 charge billed to my account should any part of my delivery container become damaged in any way. Please notify Matt's Organics, or ask driver for assistance, if you have difficulty opening container. ALL CONTAINERS REMAIN THE PROPERTY OF MATT'S ORGANICS AND MUST BE RETURNED upon cancellation to avoid additional charges. Loss: I understand that after delivery to designated address, should my produce be stolen or ruined either by weather or negligence, I am still responsible for payment of the delivery. Credits: Credits must be arranged within 24 HOURS of delivery. We will replace in your next scheduled delivery. NSF Checks: I understand that there will be a $25.00 fee billed to my account for each check returned NSF from the bank. Payment: I understand that payment is due upon delivery unless other arrangements have been made.
